General Practice Clinical Pharmacists (Durham & South Shields Area)

IntraHealth and Academic Health Science Network North East and North Cumbria are seeking bright, enthusiastic and proactive pharmacists to develop as General Practice Clinical Pharmacists in IntraHealth’s Durham and South Shields medical practices.

These positions are part of an NHS England National Programme to encourage the development of this role and as such will take part in national learning and knowledge exchange events. IntraHealth and Academic Health Science Network North East & North Cumbria are working together to develop a model to support clinical pharmacist teams and to expand the clinical pharmacist role.

You will develop your confidence through team clinical supervision sessions and benefit from direct mentoring with an experienced GP.  You will also be part of a local cluster of clinical pharmacists working under the supervision of a Senior Clinical Pharmacist.  You will be part of an organisation which has over 20 years’ experience of primary care pharmacy and is a leader in primary care medicines optimisation and is already delivering a further three clinical pharmacist pilot programmes around the country.

You will be supported to become a key member of the core general practice team and IntraHealth are committed to this role in the long term.

Within these positions you will directly improve patient care and support the efficient running of a new model of general practice. You will be primarily based in a single practice to improve continuity of care. However you will also be encouraged to develop your own clinical specialism which may benefit your practice cluster. You will become the practice expert in medicines optimisation and have input into the following areas:

  • Acute prescribing for minor ailments and common illnesses
  • Oversight of repeat prescribing requests and systems
  • Management of medicines following admission and discharge to hospital
  • Supervision of monitored dosage systems processes
  • Conducting medication review and giving advice to patients
  • Management and support of patients with long term illness as part of multidisciplinary clinics
  • Long term condition programmes e.g., frailty and bone health

We are looking for individuals with good communication and interpersonal skills who are pro-active and able to work both as part of the practice team as well as autonomously.

It would be an advantage if you were an independent prescriber but we will provide you with training and support to become an independent prescriber.

The salary will be dependant on experience and qualifications. The range is deliberately wide to encourage applications from individuals from a spectrum of backgrounds.  You will also have access to the NHS Pension scheme.

Salary range: £33,500- £42,330 p.a. WTE Full and Part time positions available
